FDA requires a new prescription even if you were already using the FreeStyle Libre 14 day system, the care team will obtain required documentation
RETURN & REFUND POLICY
New and unopened items are eligible for a refund or exchange within 14 days of the original shipment date. Because your timeframe is limited, we recommend inspecting your package upon receipt so you can address any issues immediately.
For a return to be accepted, the item must be:
- Authorized prior to shipping back
- Shipped at the customer’s expense
- In original, unopened, undamaged packaging*
- Received within 14 days of original shipment
- Within 5 months of its expiration date
A refund or exchange after receiving and inspecting the returned item will be issued in the original form of payment used to purchase the item.
If you cancel an order after it has shipped it, your order will be subject to this policy.
