What an ACO gets
- Your cohort, found. You set the criteria, for example type 2 diabetes with chronic kidney disease. Every match across your participating practices comes back for you to approve.
- Care that acts on it. PCP 1st enrolls each patient, supplies the blood pressure cuff and CGM, and its RN case managers call when a reading needs it. The patient keeps their primary care provider.
- A report you can use. Every month: enrollment, reading adherence, escalations, open care gaps, and which practices close flagged gaps.
